Ramana Maharishi sure knew how to dissociate his Atman from his physical body. So he did not need any anesthetics during the surgery.

After the removal of the avidya that this body is me , he became a jnani.
This wisdom or jnana dawned on him at a very young age. Throughout his
life he was immersed in the self and never for once he thought of the body
at all. I had the purva punya of knowing him personally.
