The OCA's Diocese of the West has an article on their website for readers, which covers some "how-to" ideas. Although online, the text is copyrighted, so perhaps it would be almost as good as a "paper and ink" article.

There is also a PDF article at chrysostom.org entitled Readers, Cantors, and Church Music in Early Christian Worship (link here), which may have some useful information.

Finally, Fr John Whiteford's website has posted Instructions ... for the Church Reader, an excerpt translated from the book The Orthodox Divine Services, Practical Guidance for Clergy and Laity (page numbers, translator's name, etc. are provided on the web page).

Out of time now, but I'll do some more poking around later and see what I can find.
