Every now and then I have similar issues with unpleasant faces intruding when I'm trying to meditate.

It took me a while to finally realize they were reflections of my own fear and anxieties.

Now if something like this happens, I take a moment to acknowledge the face (or other unpleasant image) and then dismiss it. Sometimes it's quite persistant and I have to dismiss it more than once, but it always goes away if I insist.

It might work for you, too.
