1) I'm not to judge what the best way is but plane seems to be the most comfortable (but the most expensive) means to get to Bangkok, dozens of flights are operated daily.

2) Bus is the cheapest but it's 13 hours straight. It's worth trying if you're curious to know how an astronaut feel when they return home after a long time in zero gravity space station.

3) There's no train from Phuket to Bangkok so you have to catch a bus a quarter way northeast to Surat Thani (correct me if I'm wrong) and then you will catch another train to Bangkok. Trains seems to be slower than buses in most routes (a few hours on bus from Phuket to Surat and it might take up to 12 hours from Surat Thani to Bangkok on a train) but it has more space for passengers to stretch themselves.
