hmmm... in one of Terry Pratchetts books there was a story about how Death was dismissed from it's duties and thus the world was overcrowded with "life force" that didn't pass over to the "other side" and it was leaking into reality by every option possible. lets say, when a person cursed or possibly thought of something very bad, there materialised a creature that represented the idea of the swearword or the bad thought. Pratchett's ideas sometimes are really bright and somehow this one could make a lot of sense. Like your thoughts, worries, urges and whatnot materialising in this OBE realm/twilight zone/whatever-you-call-it.

though I'm really not sure if that can actually happen. I see that Dsmoke already said about his visuals spinning off in the astral. maybe someone else has something to say on the topic?
