The NFL should make testing mandatory, expecially for the bigger guys. I am sure the NFLPA would reject it though.

When the NFL and college encourages these guys to be 300lbs- which is never a safe weight unless you are very tall- it incurrs the responsibility of what happens to those people.

The NFLPA is a pretty weak union, so I wouldn't see them as an obstacle, but I'm not so sure they would be against heart tests anyway.
