When on the VLCD he would need to monitor his blood sugar levels religiously and perhaps adjust his insulin dosing accordingly.

I am/was a Type 2 diabetic. It is controlled down with my losses and diet. I also carried the largest amount of my weight in my middle.

When I am on VLCD my morning blood sugar level is considerably lower than normal. Because of the low carb nature of the VLCD he will likely find he needs less insulin. He'll need to be really in tune with his body in regards to this.

I had a few mornings when my BS tanked. I'd never experienced it before. I was disoriented and stumbling. When I finally checked it, it was 55. I ate some strawberries and part of a tomato and it brought the level back up within half an hour. He may do well to space out his fruits from his meals or even split the fruits so he has 4 smaller snacks.
