It was a wiring problem and the fire inspectors felt that if someone had come in the day before they would have noticed the smoking outlet.
Ugh, that's even worse. It's one thing for something crazy to happen and lives to be lost. It's another when it's less of a freak accident and more of a "something that could've been prevented."
