Since you are having success when you are with your instructor, I am going to go out on a limb here and suggest you have to start practicing the mental side of your game when you are not with your instructor. I personally think this requires as much practice as the mechanics of the swing.

You have to separate the thinking part from the execution part. So go the the range and work on:
  • Chose the shot you want to hit
  • Choose a very specific target
  • Choose the club you want to hit
  • Rehearse the swing you want to make to produce the shot, no more than twice
  • Step into your address position 100% committed to hitting the shot you chose, 100% confident that you will execute the required swing, focused on your target and relaxed
  • Hit the shot without thinking about mechanics, recreating the feel of you rehearsal swing and without worrying about the outcome of the shot
  • If you were successfully, bask in the glory of your success and build up your confidence.
  • If you botched it shrug it off and forget about it.
  • Put your club back in the bag
  • Start over with a completely new shot

This sounds simple, and kind of silly, but it can make a huge difference in your on-course performance. It actually takes a lot of discipline to practice this way.
