Masha'Allah brother I do think your intentions are very noble here, you just need to 'iron out' the idea a bit and Insha'Allah we can all accomplish that together.

What I foresee to be an issue is that you basically want this one thread to be a compendium of any and every issue facing the Ummah today, plus plans of actions for everyone to help fix that issue in their locality, etc. IMO this would lead to one very confusing thread which is about "everything". For example, if I am helping with issue "X", how will I sift through the entire thread where issues A-Z are also being discussed, in addition to my issue "X"... how will the thread be organized so I can go straight to my issue without having to read 131 other posts? That's not to the say that those 131 posts would not be important, not that at all, but rather they would take away from the focus of the issue I have taken personal responsibility to work towards.

So, what I'm proposing is a separate thread for each one of these issues. But again, so many of them already exist on the forum, so not the best idea to just start making new threads left and right with "issues".

Here are some ideas off the top of my head:

- Maybe start a thread asking everyone what they think are the biggest issues facing the Ummah today. That's it. Only the "what", not the "why, when, where, how, who", etc. Not how we go about fixing them. Just a simple thread where brothers and sisters respond with a very brief reply describing what they feel is a big issue facing the Ummah today. Now you may get people saying things like "I feel the biggest issue facing the Ummah today is that by some estimates not even 5% of the world Muslim population is making salat 5 times a day" (IMO if this one sole issue is fixed, Allah's help would come to take care of everything else, Insha'Allah), others may say it's poverty, both among Muslim and non-Muslim nations, yet others may say it's lack of awareness and knowledge towards general Islamic principles which are causing our own brothers and sisters from the Ummah to act the way they do, etc, etc. These are just some examples.

- Now you take the issues which get raised and you do some research to see if they are/have already been discussed on the forum. If a thread already exists where the given issue is/was being discussed, you can try to revive that thread Insha'Allah and/or then make a new one if you feel it would serve the purpose better.

- If no thread is found discussing the issue, then you start a new one for that particular issue only, and then that issue is discussed and plans are made to address that specific issue within each brother and sister's locality, etc.

- You would need the help of the moderators to keep any of these threads 'clean' because sadly what happens more often than not is the usual name-calling and "bid'ah accusing" and "you're a this and that" and "you're a ______ (fill in your favorite title which sounds really cool but most of us don't know what it means)", etc etc. I have started some threads which I thought served a good purpose and I have ended up asking the mods to delete the entire thread because they got out of control and I didn't want to be the facilitator of some of the things people started saying to each other. Obviously that's no reason to give up on doing something but rather just something which needs to be said and kept an eye on.

Insha'Allah we can try to iron out the ideas in your OP even further from this point.

Lastly, if you ask me, though it may sound narrow-minded to some, I do feel that the main issue/problem facing the Ummah today is that we have completely forgotten about Salat (Namaz). The one who makes salat 5X a day is considered completely "out of the ordinary", even within their own families, and is given titles (sarcastically and jokingly) such as "namazi", "molvi", "maulana sahab", "pious man", etc. It should not be as such and rather the person who does not make salat should be the one who is out of the ordinary. Alhamdulillah, I've made it a point now to joke back (well, not really joking either) with anyone who calls me such names, and tell them to call me those names even more, so maybe Insha'Allah I will one day really become a "pious man" and whatever else "nice" names they call me. How can we expect any other issue to be solved when we're not even fulfilling the purpose of our creation which is to worship our Creator, Allah ? Alhamdulillah, movements like TJ are helping make a difference but I do think we could ALL (those 'involved' with TJ or not) do way, way, way more to help make a difference with this. So, to me, that would be the first issue on the list in response to your OP.

May Allah give all of us the same, intense fikr (concern) for the entire Ummah and for all of mankind as has been expressed in the OP. Ameen.
