There are general rulings

and

There are exception for mazoor. Scholars give exceptions after studying the uzar. Other recipients should not create the uzar. People with existing similar uzar can receive the exceptions.

It is something like doctor and patient relationship. How can a doctor help a patient when the patient doesn't believe the doctor.
