Assalamu alaykum

I am trying to buy this http://www.furat.com/index.php?Prog=...kinfo&id=20061 and I want to see how much the shipping will be.

However, when it asks me to enter my address, I can't seem to figure out how to enter my U.S. address into the fields... some of them seem pretty similar to me. Can anyone help me, perhaps by using the following fake address (and tell me what goes where)?
