From what I've learned, using a plastic toothbrush (or even a rough cloth, etc) fulfills the general sunnah of the action of siwak, or brushing the teeth. However, using a stick from an arak tree etc., i.e. using the miswak is a specific sunnah which is rewardable with intention. Insha Allah someone knowledgeable can correct me if I am wrong.

Also, I thought that arak tree was the usual source for a miswak but that even in the time of the Prophet صلى الله عليه that he and the Sahabah did use other tree sources occasionally. I will try to find more information about this.
