May Allah help you, it is obvious from reading this "wall" that there is some deep sincerity. I am also a Hafiz, and sometimes being Hafiz doesn;t mean one is better in their faith than anyone else. They say practice makes perfect, but you must not get too overworked and then stop. Dont get too worked up on the tajweed.. like they say.. Rome wasnt built in a day and Salaah cannot be perfected nor one's tajweed in a matter of days. it takes time. You are making a great effort, it makes me feel sad that I cannot do the same. May Allah give me your quality of Istiqamaat i.e Steadfastness.

Moreover I also think you must just simply listen to Qur'aan more.. it may help you improve your recitation... or find a teacher.
