Here is the kind of dua I do and have been advised to do:

Ya Allah!

a) Please forgive the mistakes and weaknesses in my Fajr salah and the salah of the whole of the Muslim ummah and accept our salah.
b) Please give us all taufeeq to correctly perform our salah such that it gets accepted.

Here is the dua I do after reading the Glorious Quran:

Ya Allah!

a) Please forgive the mistakes I committed in reading the Quran and please accept my reading of it.
b) Please give taufeeq to me and the whole Muslim ummah to read the Quran with tajweed.
c) O Allah! Grant me and the whole ummah*, the Guidance (Hidayah) that you have hidden behind the words of the Glorious Quran.

"Ya Allah! Quran ke alfaaz ke peeche jo tu-ne hidaayat ko chupa rakhi hai, woh hidayat mere aur saare ummat ke huqq mein naseeb farma"

*meaning even non-Muslims, you needn't say it, just imagine that and say 'ummah'.

There's one more but only used to transfer the thawab - not necessary here.

----------It looks all long when you read it, but believe me it doesn't take any time at all when you say it - around 20 seconds.

Remember to do duas with complete certitude, conviction etc.
