Dog name tags... ID discs Where can I get some?

Does anyone know where I can get dog name discs from?
I wanted two for my dogs with their names and our contact details on. Close to Cabarete please.http://www.dr1.com/forums/images/smilies/happy.gif

Dr. Bob used to have them made, not sure where, or if he had his own machine. His office is at the end of that little dirt road next to the Coastal station just outside Sosua.
