Technically, it measures the concentration of ketones in your urine.

The majority of ketones are produced by the conversion of fat into sugars or ketosis However, it is not a particularly accurate measure. Other things eg level of hydration, level of exercise, type of fats consumed, and individual liver metabolism affect the ketone concentration. Thus the ketone strips are an indirect measure of ketosis.

The general consensus is they are not worth using.
