I know what you mean by feeling hungry. I am on VLCD 4, but did not inject this morning due to TOM, but had stuck to the diet. I feel a little hungry still, but not nearly as much as I did on day one and two after gorging.

I have not seen any limit on stevia. I have it in my coffee in the morning, and on my strawberries in the evening and have lost 7lbs in 4 days, so I don't think it is hurting.

I read that if you choose to take out the melba toast that you do not replace it with anything else, even though you may not be getting additional calories from the substitution. I have read that some people have replaced it for other comparable breads, but I am too afraid to risk it, so its really a personal choice for you.

Below is the blurb I read in "Pounds and Inches" Dr. Simeons manuscript:

Making up the Calories
The diet used in conjunction with HCG must not exceed 500 calories per day, and the way these calories are made up is of utmost importance. For instance, if a patient drops the apple and eats an extra breadstick instead, he will not be getting more calories but he will not lose weight. There are a number of foods, particularly fruits and vegetables, which have the same or even lower caloric values than those listed as permissible, and yet we find that they interfere with the regular loss of weight under HCG, presumably owing to the nature of their composition. Pimiento peppers, okra, artichokes and pears are examples of this
