Vets know how to handle aggressive dogs. I would call them ahead of time and see what they say. Just explain your situation.
You could allways use the vaccine visit as a training exersize.Just a thought.

Calling ahead of time and explaining the situation (dog doesn't like strangers, will be muzzled, using this as a training exercise) is definitely a good idea. They'll know what to expect, and seeing as it's a veterinary practice, they probably have some protocol for dealing with a difficult dog. Plus they can have everything ready to make it as quick as possible.
