Cut down on your carbohydrate and raise your protein intake, make sure to cut down on the amount of saturated fat and alcohol that you consume, and aim for at least one 30mins period of hard grind/sweat a day.
Walking, Swimming, Rowing and Weight Training are all good for fat burning, and actually drinking a fair amount of caffeine (but not too much) helps too as it raises your metabolism levels slightly. But seeing as you seem to be doing enough excercise, it's probably your diet that it giving you problems. It might be worth checking to see if your adversely intolerent to a particular type of food, for example if I eat too much bread I put weight on, no over type of food seems to have this effect on me, until I went to the doctors and found out I had a yeast/gluten intolerence (not allerigic, but my body doesn't break it down properly).
Might be something worth looking at.
Some good 'fat burning' foods :-
Berries
Strawberries, raspberries, blackberries, blueberries, black and red currants all stimulate the main organs that are involved in detoxification, boosting metabolism and making the body more efficient at burning fat.

Drinking it will also raise your metabolic rate and speed up fat oxidation, so helping you lose weight.
Olives and Olive Oil
Certain fats are good for you and your body needs them. And olive oil is one of those "good fats". In fact it's so good that it actually helps you burn fat and keeps your cholesterol down. However, it should be used in moderation because an excessive amount will have the opposite effect.
Eggs
The white of an egg is very high in protein and essential in helping your body break down and burn fat. Eggs will also help you feel full for longer and decrease your appetite.
Pineapple
Eating fruit in the morning helps you digest your food more efficiently during the day, and pineapple contains special digestive enzymes that work double time to help you digest food quickly.

Celery
Use celery to bulk our your meals. Plus, it is so low in calories that you actually burn up more munching your way through it than you gain when you've digested it.
