It can be any part of the head, and it doesn't have to be a hit, it can be caused by a violent shake too. In simplistic terms it's caused by sudden movement of the brain causing rotational forces or twisting forces in the brain tissues. Not sufficient for physical permanent damage, but enough to affect normal functioning to a greater or lesser degree. Usually rights itself in 2-3 weeks depending on seriousness.
