My understanding is that the prevalence of desomorphine in Russia is due to less restriction on the sale of codeine (its precursor). Fortunately here in the States, desomorphine is rarely seen (according to those I know in the toxicology field at least), and I would have to attribute it in large part due to the controlled nature of codeine.
