For me everything is played off my lead ear except driver.Driver is played inside front heel.
The width changes per club in hand.Driver ,3wood and hybrids -width is where insteps are just outside shoulder width.Long irons half foot narrower.Mid irons full foot narrower.Short irons 1 1/2 foot narrower.

Reason why I pick off ear, besides that's what I was taught- is something needs to be consistant with the swing. So,ball position it is.

Woods- ball closer to left heel than center, but more center than some
All Irons- middle of stance (unless I am doing some kind of shot-shape, hi, low etc)
Wedges - middle (same as irons on shot shape)
Same here although I am not shaping shots yet. as far as my stance goes my feet are shoulder width apart.

The bigger the club the closer to my left heel the ball is. My feet tend to be shoulder width apart for most shots as well.

Driver - Inside front foot
Irons - Used to be front foot, but somehow it changed to middle and now it feels uncomfortable at the front. Now I am pushing it badly.
Wedges - Front foot.

I played yesterday and started double crossing again. So today I put the ball visually, maybe one ball ahead of center with the driver and got my fade back (most of the time). Once I get brave I will put it two balls ahead of center and I may hit it straight. I just wanted my controlled fade back!

Driver about 1" inside left heel
Progressively further back towards the right foot from there. Wedges are in middle of stance.

If i play it more forward than that, I get inconsistent. My weight shift is not as good as is should be.

Default Ball Position & Stance Width
Curious to know where and how you use ball position and your stance vs woods, long irons, short irons and wedges.

When I take my stance for regular full swing shots, the ball is inline with the inside of my left heel, maybe just a tad back in my stance. The two exceptions are specialty shots, or when using my driver. My driver ball position a little more forward. Specialty shots may require a different ball position, or stance due to situation at hand. Once I get my left (forward) foot set, I adjust my stance width by moving my right (rear) foot forward, or backwards. My stance width takes care of the different club lengths, and is part of my pre-shot routine.

My stance is a little wider than shoulder width. I like to use my legs in the swing and this helps me have better balance.

The ball position is typically in the center of the stance or just slightly ahead or behind center. This is with any club. I pretty much use the same ball position for a 4-iron that I do with a wedge. Even my driver I play pretty close to center. I have had the thing for 6 years now and that is just the best place to play it to get the maximum use of the shaft flex. If I play it too far forward it is hook city. The only club I will make a concious effort to play more towards the front foot is my 3-wood. It has a slightly stiffer shaft so I need a couple extra inches to get the face square.

This is all under normal shot conditions. Hills, uneven lies, flop shots, hooking a ball around a tree while keeping it under the branch, things like that, I will position the ball how I need to hit the shot.

Actual stance is shoulder to very slightly larger than shoulder width. Toes are in/out/straight to target, it varies and again depends on what I am trying to do with the shot.

Weridly enough I like to chip further back in my stance than most, I find that when I do miss-strike my chips occasionally, I get a little more forgiveness this way.
