ISLAM:
1. to declare LA ILAHA ILLALLAH MUHAMMADUR RASOOLALLAH
2. to perform the obligatory salats
3. to perform the obligatory fasts
4. to give zakat (if eligible)
5. to perform hajj (if eligible)

EEMAN:
To have strong Belief in
1. ALLAH
2. MALAAIKAH
3. Allah's Books
4. Allah's prophets
5. The day of resurrection
6. gotaqdeer is from ALLAH
7. Aftermaths of death.

What is the difference between Imaan & Islam?
What is Iman?
There are three stages of belief in Islam: Islam, Imaan and Ihsan. For example, Islam is the mere physical movement in prayer. Imaan is the mental belief in prayer. Ihsan is the spiritual belief in prayer. From the Ihsan are the Muhsin (the good doers). So, one must strive to be a Muhsin in his lifetime to recieve the best blessings of Allah. That is just a short description of it.
