However, when it asks me to enter my address, I can't seem to figure out how to enter my U.S. address into the fields... some of them seem pretty similar to me. Can anyone help me, perhaps by using the following fake address (and tell me what goes where)?

Well, the thing is, I know what the words mean but some of them are similar in meaning, and I'm not sure how an American address would fit in there. I've tried a few combinations but it wouldn't give me a shipping price or get past that screen.

fifth is street address
sixth is zip code
seventh is phone number.
