The "dalil" was provided in my first post. The part between brackets is also from the Reliance. You will find this in most shafi'i fiqh books. Here is a passage from Bushra (from the matn and some parts from the hashiya):
ولو حضره ذكران صفا خلفه وكذا إذا حضرت المرأة وحدها فتقف هي خلفه ولو محارم للاتباع |

Good question. In the books the scholars made a general statement saying it's recommended for a woman to make iqamah for her self or for a group of women. As far as I have seen or heard there is no exception, not even for the husband. It seems a little bit strange though, because the reasons ('illal) for not doing adhaan are not present in the iqaamah. Maybe a shafi'i scholar can answer this question.
