Dear All
You must be aware about "Unique Identification card" being given to all by Govt of India. In future this card will serve purpose for identification proof for everybody. Following are the points you should know. Card will be given for above 18 years with the proof of identity and residence. From 5 years to 18 years card will be given only if original birth cert. isа available otherwise parent should have UID card. Govt. has opened centres in all localities. You can go to http://uidai. gov.in/ аto know the centers which will be nearer to your residence and you can also download the form from there also. There is a long queue for getting form at the center. People stand from morning 5 a.m. for hours. ONE FORM IS REQUIRED PER PERSON, all members have to be present to have the forms Get the FORM fill in bold letters. Attach Xerox of your identity and residence proof. Also Xerox Copy of PAN CARD and VOTERS CARD or any other documents as mentioned in the website, Ration card is also required. Go to the centre with originals and Xerox also. Some centres are open on Sunday also. There will be big queue. They will take your photo, finger prints and your eye Scan. It will take 10-15 minutes per person. They will give you receipt with your photo card, It will be delivered to your home within six months. |

Hi Ganpat Sir,
According to the information you have provided if the Form is available online for Download, why do people have to stand in a queue from 5am. You also say all members have to be present to have the forms but why then is it made available for download? I feel there may be others who may be wanting answers to what may confuse them. I feel the Govt. introducing new things is adding only a new queue to existing ones and inconvenience people. Rajpriya |

Dear Rajpriya Sir,
There seem to be two aspects. One is getting the UID application forms and the other is to submit the filled up application and getting tested for biometric parameters. If any forum memeber could provide the practical experience of knowing the centre via internet ( the website uidai.gov.in does not contain centre details), and how they downloaded the application form, it would be very useful. Dear Rajpriya Sir, Hi Santhanamravi Sir, Whether it is the Government or other people who want to provide useful information to assist the public it must contain lots of accuracy. I have gone through a system of Biometric Identification in Germany so I am not new to it. To make inroads into the world of “world wide web”(www) system to access information one needs skill as well. I have attached the Form herewith and I hope people who need it can access it from Chennai forum. This could be a unique service the Chennai forum could provide or from this website Aadhaar Enrollment Form By the way after reading your reply, in fact I tried to go the site with link you had mentioned but I failed. Then I found the correct web address to this site is: UIDAI? Going through it I found lots of information about the Governments’ intention is to help the poor. I know that getting the form is the first step and the second step is to be present physically at the institution where you undergo a scanning system for your fingerprints that represent the Biometric information on the ID. I hope the information helps every one on and outside the forum. Rajpriya Ps.
